I'm not sure about the car issue, the only way (barring platform bugs) it could do that is if the media button play command was sent and MSC was the last app responding to media buttons. It's not doing it on the head units and devices I've tested it…

Yes, it uses the MediaStore playlist system in Android (which allows the playlists to be accessed from other apps) which doesn't support mixing audio and video types unfortunately.

It's the 'floppy disk' icon at the top right of the screen in the action bar. On a small screen it would be contained in the 'three dot' menu as 'Save to file'.
